HIV and Hepatitis C (HCV) Screening and Health Care Access.
HEPPAC partners with primary care clinics in our community to provide a path to health care access for recently diagnosed and out of care HIV and Hepatitis C individuals.
HEPPACs screening and linkage activities include the following:
- HIV and HCV care and treatment appointment scheduling
- Patient navigation/advocacy within medical systems
- Linkage to health care coverage
- Client transportation to scheduled appointments
- Referral services
- Basic needs services.
